Required Documentation: Application Form Proof of NSNA Membership Card Proof of a cumulative GPA of 2.5 or above One (1) letter of recommendation, preferably from a nursing facility One-page statement with a brief description about yourself, reason for career choice, and any relevant HISNA activities
